Manage Your Diabetes

Diabetes Self-Management Education and Support (DSMES) services empower people to live well with diabetes. DSMES helps you to:

  • Set and track your own health goals
  • Practice how to fit diabetes self-care behaviors into all parts of life, like healthy eating and problem solving
  • Learn how to use knowledge, skills and tools to build confidence and emotional strength to manage diabetes
  • Find ways to get support from family, friends, community and your health care team

Diabetes Self-Management Education and Support (DSMES) services empower people to live well with diabetes. DSMES is covered by Medicaid, Medicare and most private insurance plans.

Diabetes Prevention

Prediabetes

1 in 3 American adults has prediabetes, and most do not even know they have it. Having prediabetes means your blood glucose (sugar) levels are higher than normal—but not high enough to be diagnosed as diabetes. Prediabetes can lead to heart disease, stroke, and type 2 diabetes, the most common form of diabetes. Prediabetes can often be reversed.

Preventing Diabetes

The National Diabetes Prevention Program is a CDC-recognized program proven that through healthier eating, increased physical activity, and a small amount of weight loss, it’s possible to prevent or delay the development of Type 2 Diabetes.

Take Action

As part of a CDC-recognized diabetes prevention lifestyle change program, you will work with a trained Lifestyle Coach and other participants to learn the skills you need to make lasting changes.

Heart Health Improvement

Cooking Matters

Cooking Matters is a six week cooking based nutrition education program that provides the skills, knowledge and confidence needed to identify and prepare healthy and affordable meals at home.

Colorado Heart Healthy Solutions

Colorado Heart Healthy Solutions (CHHS) uses a statewide network of health navigators to test participants’ blood pressure, blood sugar, weight and cholesterol at no cost. They also provide individuals with personalized health education, coaching, and referrals to local medical and healthy living resources to help improve their risk of heart disease.

CHARLAR

CHARLAR is a shared community program for adult Latinxs to learn, build skills, and improve health behaviors to lower their chances of getting heart disease. CHARLAR consists of 11 weekly classes facilitated by bi-lingual community health workers, as well as two health screenings and counseling.

Home Blood Pressure Monitoring

These programs teach participants how to check their blood pressure at home and provide ongoing assistance for at home monitoring. Other services such as one-on-one check-ins with a community health worker, nutrition education seminars and blood pressure management toolkits are also included.

Nutrition Education

Programs that provide classes and informational sessions to a variety of age groups on topics and skills related to nutrition and healthy living.

Cardiac Rehabilitation

After a heart attack you need to get your heart healthy again. Cardiac rehabilitation is a structured service that includes medical assessments, personal supervised exercise plans, a plan to make your heart health better, education, counseling, and behavioral programs.

Medication and Pharmacy Services

Medication Therapy Management

Medication Therapy Management (MTM) includes multiple services to help you and your pharmacist make sure that your medications are working to improve your health. MTM services include a comprehensive medication review, education about your medication and health, coordinating care with your doctor and more.

Insulin

The cost of medicines to treat diabetes, like insulin, may be expensive and difficult to afford each month. There are resources to help you find an affordable option for insulin.
